North Carolina officials are investigating a fatal accident at a Peninsula Packing Co., facility in Wilson, N.C., on Feb. 18 that left one man dead.
The Wilson Police Department said 44-year-old Tony Ballew was trapped inside a production machine. The preliminary report from the North Carolina Department of Labor's Occupational Safety and Health Administration said Ballew was trapped in an extruder, but investigators have not determined what caused the accident.
Peninsula Packing, based in Exeter, has both extrusion and thermoforming at Wilson for food packaging.
OSHA investigators will inspect the equipment, review safety and health training and interview witnesses as part of the full report, expected to take about three months, officials said.
