Composites supplier Continental Structural Plastics Inc. faces proposed penalties of $107,168 after an investigation by the U.S. Occupational Safety and Health Administration found employees at the company's facility in Carey, Ohio, were exposed to potential amputation and struck-by hazards.
CSP is owned by Japanese materials firm Teijin Ltd. and manufactures lightweight composites and other materials for the automotive, heavy truck, HVAC and construction industries.
The company is being cited by OSHA for one repeated and three serious violations, which include failing to adequately train workers on lockout/tagout procedures and ensuring locking devices were attached to the machine as required, as well as for exposing workers to struck-by hazards from improperly stored materials.
OSHA said it cited CSP, which is headquartered in Auburn Hills, Mich., for similar violations in 2015.
